HOPE for Elderly Independence-is a program designed to save frail Seniors from becoming prematurely institutionalized by providing the support and resources necessary for independent living.  HOPE’s mission is to enable elderly; at-risk participants remain comfortably in their own residences for as long as possible.

What the Program Offers

  • Case management
  • Housekeeping help
  • Home-delivered meal referral
  • Emergency Response System membership
  • Referral for informal caregiver’s support.


Participants must:

  • be 62 or older
  • be frail or elderly
  • be in need of in-home support
  • have little or no local support network of family and friends
  • reside in San Leandro, San Lorezno, Hayward, Castro Valley, Union City, Fremont, and Newark.
  • co-develop and implement an Individual Plan of Care (IPC) with a case manager
