HOPE for Elderly Independence-is a program designed to save frail Seniors from becoming prematurely institutionalized by providing the support and resources necessary for independent living. HOPE’s mission is to enable elderly; at-risk participants remain comfortably in their own residences for as long as possible.
What the Program Offers - Case management
- Housekeeping help
- Home-delivered meal referral
- Emergency Response System membership
- Referral for informal caregiver’s support.
Participants must:
- be 62 or older
- be frail or elderly
- be in need of in-home support
- have little or no local support network of family and friends
- reside in San Leandro, San Lorezno, Hayward, Castro Valley, Union City, Fremont, and Newark.
- co-develop and implement an Individual Plan of Care (IPC) with a case manager
